Music Forms

Please download & complete the form corresponding to your event.

Forms must be sent in to our office 2 months prior to your event date. Please be sure to put your name and event date on all forms, and in the subject line of email if you scan and email them to us.

There will be a $100.00 late fee added to your contract if all the documents and pictures are not sent in to us 2 months prior to your event.

Bar / Bat Mitzvah Forms

Instructions

You may submit all of your documents using any of the following methods:

  1. Scan and email completed forms to sales@afterhoursent.com. Be sure to put your name and event date in the subject line.
  2. Send us the original forms via snail mail (US Postal Service) to our address below. Please do not put "signature required" on package. Send regular mail with tracking (make sure you make copies of the forms for yourself).
  3. Fax forms to us at (631) 206-9144. Put a cover page with your name and event date and how many pages you are sending over.

To ensure that your documents are legible with no errors, please complete the following:

  • Use our forms and only our forms
  • Use black pen only (no pencils please)
  • Print only - no script
  • If you wish to type up a separate song list of what you want to hear, please do so in a Word / text document. Please write your name(s) and event date on it and send along with our forms.
  • Please do not use Excel / spreadsheet documents
